Champions plays the teleportation-is-deadly mechanic in a kind of funny way by skewing it twice in the characters' favor while still allowing death on rare occasions and unconsciousness on more common ones.
First, it forces teleporters to succeed on a to-hit roll, which—unless you can't see where you're going—is greatly skewed in your favor (if you buy the stats they expect). However, if you miss (which you have a 50/50 chance of doing blind even with those stats), death is still only about a 20% chance; injury is another 30% or so, and the rest is mostly falling unconscious with ridiculous overstun.
